We use the information that you have given us in order to keep a personalised up to date record.

How we store your personal information

Your information is securely stored securely on a password protected practice computer system and medical histories are in a locked manual filing cabinet. Your information cannot be access by anyone who does not work at the practice. All staff know their legal responsibility to main confidential and to follow practice policy to ensure this.

We will only retain your personal information for as long as necessary to fulfil the purposes we collected it for, including for the purposes of satisfying any legal, accounting, or reporting requirements. Patient data is retained for a minimum of 11 years, or for children until age 25, whichever is longer.

It is recognised that there are often practice difficulties in storing study models or working models, surgical guides and wax ups and it reasonable to make a decision to retain these for a shorter period of time.

Your data protection rights

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances. However dental records cannot be erased.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
